Superform provides wellness and performance optimization services. It is not a medical provider and is not HIPAA compliant. Any medical assessments or diagnostic testing associated with our programs are conducted exclusively through licensed third-party medical partners that maintain their own HIPAA-compliant systems.

Personal Information: name, contact details, date of birth, billing information, and communications.
Performance & Wellness Information: training data, sleep and recovery metrics from wearables, intake forms, and program questionnaires.
Medical Assessment Summaries: limited diagnostic or laboratory results shared by our HIPAA-compliant medical partner with your written authorization.
Technical Information: browser type, IP address, cookies, and analytics data to improve website performance.
Payment transactions are processed securely by third-party vendors; Superform does not store complete credit-card numbers.
3. How We Use Information

All laboratory testing, medical consultations, and diagnostic services are performed by independent licensed medical partners operating under their own compliance programs.
Participants authorize those partners to share specific data—such as baseline and outcome summaries, clearance status, or red-flag indicators—with Superform for the limited purpose of designing and administering wellness and performance programs.
Superform receives and stores this information securely but does not create, maintain, or manage complete medical records.
5. Data Sharing
We may share limited information with:
Authorized contractors, trainers, or technology providers who assist in program delivery
Licensed medical partners as described above
Legal authorities if required by law
All such parties are bound by confidentiality or service agreements restricting secondary use or disclosure.

Superform uses secure cloud infrastructure, encryption in transit, password protection, and access controls to safeguard information. While we maintain commercially reasonable security measures, no online system is completely risk-free.
